I welcome the release of NHS England’s recent report ”Where Best Next”, which David Oliver writing for the BMJ also endorses. Indeed, it has led me to reflect on the issue of what is “a timely discharge?” A poster or leaflet telling NHS staff how to, and what to do regards reducing length of stay will require more than visual prompts. It requires a whole society culture change. Society has changed so much over the years. Men and women are working longer so cannot provide consistent care support for their relatives. Families no longer live local to relatives, and social services ability to monitor those who are vulnerable is frequently stretched due to reduced resources or simply the numbers on their caseload.
